1. SUBJECT
Final Acceptance of Bid No. 189-04; SP-4048, Exterior Painting at the Main and Branch
Libraries and authorization to file Notice of Completion with the County Recorder.
2. SUMMARY
The project included painting the exterior building at the Main, EI Modena and Taft
libraries. Other aspects of the project included repairing damaged walls, painting pillars,
and planters.
3. RECOMMENDED ACTION
Accept as complete Bid No. 178-04; SP-4048, Exterior Painting at the Main and Branch
Libraries and authorize staff to file Notice of Completion with the County Recorder.

NOTICE OF COMPLETION AND ACCEPTANCE
NOTICE IS HEREBY GIVEN that work on that certain public work and improvements known as"Exterior Painting
at the Main and Branch Libraries.(Bid No. 189-04;SP-4048)"(herein referred to as the"Project"),for the owner,the
City of Orange,a municipal corporation(herein referred to as the"City"),whose address is 300 E.Chapman Avenue,
Orange, CA 92866, was completed and accepted by the City Council of the City of Orange on April 9, 2019 at a
regular meeting of its members, at which a quorum was present and acting throughout. The Project is at Main, El
Modena and Taft libraries in the City of Orange, State of California. The contractor who performed(or caused to be
performed)said public work and improvements were US National Corp., a California corporation,with its principal
o�ce at 10205 San Fernando Road,Pacoima, State of California.
The name of the surety on the Labor and Material Bond for the Project is Indemnity Company of California(Bond
No.380211P)in the amount of$54,200.00.

reimbursements from State of California Office of Emergency Services and authorized the

Grant Americans with Disabilities Act Wheelchair Access Ramps at Various
Locations Project Fiscal Year 2018-19; Sid No. 189-21. (A2100.0; AGR-6754)
ACTION: Awarded the contract in the amount of$207,910 to All Cities Engineering,Inc.
of Jurupa Valley, California and authorized the Mayor and City Clerk to execute on behalf
of the City.
3.17 Award of Contract to All American Asphalt for Annual Pavement Maintenance at
Various Locations Project Fiscal Year 2018-19; Bid No. 189-23.(A2100.0;AGR-6755)
ACTION: 1) Authorized the transfer of $275,000 from 263.5011.56330.20187, Tustin
Street Rehabilitation to 263.5011.56330.13120, Pavement Management Program; 2)
Awarded the contract in the amount of$5,369,997 to All American Asphalt of Corona,
California and authorized the Mayor and City Clerk to execute on behalf of the City; and
3)Authorized the City Manager to approve contract change orders up to 5%of the contract
amount.
