(FY18).
2. SUMMARY
The City of Orange has entered into a contract with Vido Samarzich, Inc. for the construction of
the (CDBG) (ADA) Wheelchair Access Ramps project. The work is complete to the satisfaction
of the Public Works Department.
The project provided for the construction of 48 ADA wheelchair access ramps at various
locations, citywide. The work also included replacement of failed sections of curb and gutter,
sidewalks, cross gutters, spandrels and detectable warning surfaces (truncated domes) per the
American with Disabilities Act Accessibility Guidelines (ADAAG) requirements.
3. RECOMMENDED ACTION
1) Accept Bid No. 178 -20; SP -4027; Community Development Block Grant (CDBG)
Americans with Disabilities Act (ADA) Wheelchair Access Ramps at Various Locations
Project (FYI 8) project as complete.
2) Authorize staff to file Notice of Completion with the County Recorder.

NOTICE OF COMPLETION AND ACCEPTANCE
NOTICE IS HEREBY GIVEN that work on that certain public work and improvements known as "CDBG ADA
Wheelchair Access Ramp Replacement at Various Locations Fiscal Year 17 -18 (Bid No. 178 -20; SP- 4027)" (herein
referred to as the "Project "), for the owner, the City of Orange, a municipal corporation (herein referred to as the
"City "), whose address is 300 E. Chapman Avenue, Orange, CA 92866, was completed and accepted by the City
Council of the City of Orange on March 8, 2016 at a regular meeting of its members, at which a quorum was present
and acting throughout. The Project is at Various Locations in the City of Orange, State of California. The contractor
who performed (or caused to be performed) said public work and improvements were Vido Samarzich, Inc., a
California corporation, with its principal office at 4972 Crestview Place Alta Loma, State of California.
The name of the surety on the Labor and Material Bond for the Project is Western Surety Company of California
(Bond No. 08712161) in the amount of $204,300.00.
