2. SUMMARY
The following claim(s) have been received and investigated by the City Attorney's Office
and/or the Risk Manager. After a thorough investigation with the involved departments, it is the
recommendation of the City Attorney's Office that the claim(s) be denied:
a. Kara Harty
b. Kirk Harty
c. Emerenciana Pahuamba Sanchez
d. Progressive Insurance subrogee for Ashlei Peckels
e. Daniel Jameson
f. Martha Carrillo
3. RECOMMENDATION
Deny claim(s) and refer to City Attorney and Claims Adjuster.

17. DISCUSSION and BACKGROUND
a. Claim for damages from Kara Harty alleging wrongful death as a result of a
motorcycle accident occurring outside of City boundaries.
b. Claim for damages from Kirk Harty alleging wrongful death as a result of a
motorcycle accident occurring outside of City boundaries.
c. Claim for damages from Emerenciana Pahuamba Sanchez alleging personal injury
during a traffic accident on an OCTA bus while riding as a passenger.
d. Claim for damages from Progressive Insurance subrogee for Ashlei Peckels alleging
property damage during a traffic collision with a City vehicle.
e. Claim for damages from Daniel Jameson alleging property damage to his vehicle
during a high wind event.
f. Claim for damages from Martha Carrillo alleging property damage caused by tree
roots.
