This Support Dog Return and Release Agreement (“Agreement”) is entered into as of June
____, 2025 by and between the City of Orange, a municipal corporation, (“City”) and Guide Dogs
of the Desert, a California nonprofit corporation (“Guide Dogs”).
RECITALS
WHEREAS, Guide Dogs previously provided a support dog identified as Lily, an English
Yellow Labrador with microchip 933000320870617, (“Dog”) to the City’s Police Department
(“OPD”) for the wellness and support of its personnel.
WHEREAS, OPD has determined it is appropriate to return the Dog to Guide Dogs at this
time.
WHEREAS, the parties acknowledge and agree that Guide Dogs will provide a suitable
replacement support dog to the Department, at no cost, at a future date to be determined by OPD.
NOW, THEREFORE, IT IS HEREBY AGREED BETWEEN THE CITY AND
GUIDE DOGS AS FOLLOWS:
1. Return of Dog. The City hereby transfers custody and possession of the Dog back to
Guide Dogs, effective June ____, 2025. Guide Dogs accepts the Dog in as-is condition and
assumes all responsibility for its care, custody, and control as of the date of transfer.
2. Ownership and Liability. Upon transfer, Guide Dogs shall be the sole owner of the
Dog and assume full responsibility for its health, safety, and conduct. The City shall have no further
obligations or liability related to the Dog, and shall not be held responsible for any incidents,
injuries, or claims arising from the Dog after the date of return.
3. Release and Waiver. Guide Dogs hereby releases and forever discharges the City,
OPD, and their officers, employees, and agents from any and all claims, liabilities, losses,
damages, or causes of action of any kind related to the Dog after its return.
4. Replacement Dog at No Cost. Guide Dogs agrees to provide a suitable replacement
support dog to OPD at no cost, upon OPD’s written request and timing.
5. Entire Agreement. This Agreement contains the entire understanding between the
parties and supersedes any prior agreements, written or oral, regarding the subject matter herein.
6. Governing Law. This Agreement shall be governed by and construed in accordance
with the laws of the State of California.
7. Counterparts. This Agreement may be executed in counterparts, including by
electronic signature or scanned copy, each of which shall be deemed an original and all of which
together shall constitute one instrument.

IN WITNESS, the parties have executed this Agreement as of the date first written above.
