SUBJECT Contract Change Order No. 1 - Bid No. 145 -31; SP -3904; Annual Concrete Replacement at Various Locations Phase 2, Fiscal Year 2014 -15. 2. SUMMARY This Contract Change Order authorizes payment for extra work at agreed prices. The extra work consists of additional concrete repair work on sidewalk, curb and gutter, ADA wheelchair access ramps, and driveway at various locations. The total cost for this change order is $99,850.00 3. RECOMMENDED ACTION Approve Contract Change Order No. 1 and Final in the amount of $99,850.00 to C.J. Concrete Construction, Inc. for the extra work. 4. FISCAL IMPACT Funds are budgeted and available to cover the cost of the Capital Improvement Program for FY 14 -15 as follows: 263.5011.56330.13120 (Local Street Maintenance) $ 99,850 5. GENERAL PLAN IMPLEMENTATION Infrastructure Element Goal 3.0 Ensure adequate maintenance of public rights -of -way to enhance public safety and improve circulation. Policy 3.1: Continue to maintain and repair sidewalks and pavement surfaces on public rights -of- way. 7. DISCUSSION and BACKGROUND The City Council awarded the Annual Service Contract for Concrete Replacement Phase 2 at Various Locations, FY 2014 -15 on March 10, 2015 to C.J. Concrete Construction, Inc. The unit bid prices received from C.J. Concrete Construction, Inc. were very favorable. As such, Staff recommends adding approximately 12,000 square feet of sidewalk, 1,600 linear feet of curb and gutter, 1,200 square feet of driveway approaches, and 4,500 of square feet of ADA wheelchair access ramps replacements to this project. The estimated total quantities of concrete replacement including this change order for this project are as follows: 1. 41,000 square feet of sidewalk on arterial and residential streets 2. 5,000 linear feet of curb and gutter on arterial and residential streets 3. 4,000 square feet of driveway approach on arterial and residential streets 4. 18,000 square feet of ADA wheelchair access ramp on arterial and residential streets The original contract amount was $589,300. This Contract Change Order represents a 14.5% increase in construction costs to $689,150. 8.
