Exposure - Consent to Provide Blood Sample Form
In the event of an exposure, or suspected exposure, to blood or the bodily fluids of an arrestee/subject,
please inform your supervisor immediately. Employee health is our priority and the objective is to try and
obtain a voluntary blood sample from the arrestee/subject, prior to their release. If the subject consents to
providing a blood sample, employees shall utilize the Exposure – Consent to Provide Blood Sample
document (OPD E-5) which can be found in the Form Room. If the subject refuses or is released in the
field/booked at OCJ, the Department of Health Services is responsible for obtaining, or trying to obtain a
sample, which may or may not occur.
The sole objective for obtaining the voluntary sample is to have it analyzed for Human Immunodeficiency
Virus (HIV), Hepatitis B (HBV), and Hepatitis C (HCV). The quicker the sample is obtained, the more
we can be assured our officer(s) are afforded the opportunity to receive appropriate medical treatment.
This form shall not be used for any other purpose.
The form is available in both English and Spanish. The person providing the voluntary sample should
read the form in its entirety before signing it.
Bottom Line: In the event of an exposure, or suspected exposure, to blood or the bodily fluids of an
arrestee/subject, officers should immediately inform their supervisors. If the arrestee/subject consents
to a blood sample, the Exposure – Consent to Provide Blood Sample form (OPD E-5) shall be used for
the sole purpose of having it analyzed for HIV, HBV, and HCV.
